Patient & Family Partnership Council
As a Primary Care Medical Home, we want input from YOU, our patients.
Keller Army Community Hospital (KACH) plans to conduct Patient & Family Partnership Council meetings on a quarterly basis. Patient & Family Partnership Council is a group of individuals consisting of current or former patients and family members - that have received care at KACH - and hospital staff. The goal is to partner, in a positive way, to affect operations and policies, and to enhance the hospital experience for all patients and families who receive their care at KACH.
The focus of the Patient & Family Partnership Council includes but is not limited to:
- Ensure the voice of the patient is represented.
- Offer valuable insight to leadership and the care team.
- Facilitate a stronger physician-patient relationship.
- Improve patient education and adherence.
The Patient & Family Partnership Council is open to all Keller beneficiaries, including active duty, reservist, veterans, retirees, and the Greater West Point community - including all West Point employees and dependents.
Please consider joining in and bringing your input, thoughts, ideas, concerns, and/or praise to our meetings. We welcome the opportunity to listen to the thoughts and ideas from the community we serve in an effort to make Keller Army Community Hospital a better place for those who receive and provide healthcare.
The Patient & Family Partnership Council is led by the Patient Advocate.
