Accreditations and Credentials

Patient Identification and Demographic Verification Policy

All patients must present valid DoD or VA identification and verify their full name and date of birth before receiving any medical services. Additionally, demographic information will be verified to assure patient’s information in MHS GENESIS is accurate and up to date.

This policy is in effect for your safety and to ensure accuracy of medical records.

All minor patients – over the age of 10 – are required to possess and present their own DoD ID card.

Patients who arrive at their appointments without their DOD or VA identification card will be sent to the Admissions & Disposition (A&D) office (Keller, Second Floor, Rooms 2E02 and 2E03) to confirm eligibility prior to being checked in.

Patients must provide a valid government-issued ID or passport to the A&D staff, as well as their SSN or DOD ID Number; their SSN or DOD ID Number will be used to perform a Defense Enrollment Eligibility Reporting System (DEERS) check.
